Each January every Knights of Columbus Council submits a Fraternal Activity Report to the Supreme Council in New Haven CT.  These data reflect activity in a calendar year and are used primarily for national and international publicity.   In 2011, for example, 1.8 million knights reported over 70 million volunteer hours of community and related service along with 413,000 blood donations.

This form is designed primarily for knights who do not have Internet access.  If you have access you should go to Member Profile under MEMBERS and record your charitable volunteer hours on that screen.  By using the on-line option, the information will be available for you to review and to update on an ongoing basis. It will also be available for end–of-year reporting as referenced earlier. 

Use this form if you are a knight:
  1. Who does not have internet access so you can manually record all of your charitable volunteer hours and can accurately complete the Fraternal Activity Report at the end of the calendar year.
  2. Who has internet access but who wants to keep a manual record temporarily and update the Member Profile electronically on a periodic basis.
Record all of your charitable volunteer hours, including those of your spouse when she works with you on a project.  This reporting is not affected by any other volunteer activity forms you may have submitted to the local council, assembly, or the state council.  This information is cumulative and is being reported to a separate entity – the Supreme Council. 

However, do not record time participating in, or preparing for, council and assembly meetings and dinners, and exemplifications.

Assembly Members
By checking the 4th Degree box, assembly members can identify patriotic volunteer activities including: performing civic duties at the village, city, county, or state level; honoring the flag; assisting veterans, and placing the flags at Sacred Heart of Jesus etc. in the Community category.  The hours for Honor Guard activities would typically be either Church or Community.

Category Explanation
Church Hours spent in any Church related activities/ministries.
Community Hours spent in any community related activities, including preparation and conducting fundraising activities.
Youth Hours spent in any youth related activities.
Blood Donor Hours spent donating blood (make a separate entry for each donation).
Habitat for Humanity Hours spent in building homes and related activities.
Sick-visit Hours spent with a sick person at home, nursing home or hospital (make a separate entry for each visit).
Sick-provide service Hours helping a sick person by performing household chores, preparing meals, transporting, and counseling, etc).
Condolence visit Hours spent in visiting or helping a bereaved family and attending a wake service or mass etc.  (make a separate entry for each visit).
Miscellaneous Hours spent in service not previously described, including some council activities that will benefit a charity or a cause.
