This form is designed for knights who do not have Internet access.  If you who do have access, you should go to Member Profile under MEMBERS and update your interests on that screen.  By using the on-line option, the program directors and activity chairs will have immediate access to your information.

To complete the form manually:

1. Enter all of the requested information at the top of the form.
2. Click the box preceding each activity in which you are interested.
   1. Checking an interest does not mean that you are obligated to participate in that event the next time it is scheduled – it means that you are interested and will help, if available.
   2. This information will be used by the program director, individual directors, and activity chairs to contact you when the event is scheduled and ready for implementation.  You should expect to be contacted prior to each time the activity occurs, and you can then choose how to respond.
3. Submit the completed form ASAP to the Program Director, who will enter the information into your record.
4. Keep your Member Interests current by updating your record on an ongoing basis.
