This form is designed to assist the Activity Chair in recording and reporting the number of knights and the number of cumulative hours (including spouses) that were spent on a specific activity.  Reports to the State Council typically ask for: a) the number of knights involved in a given project; b) the percentage of the council membership it represents; and, c) the cumulative number of hours involved in the project.

The information recorded on this document will serve three primary purposes:

1. As a planning tool for knights who will coordinate and implement the same activity when it is again scheduled;
2. As a reporting tool for the Program Director who must keep a record of all fraternal year programs in order to help accurately complete (as previously referenced) the forms required for the State’s Star Council Award and other awards.
3. As a summary report to the director, who can provide ongoing and year-end reports on the progress of activities under his leadership.

Reporting on this form does not negate the need for individual knights to complete their own Volunteer Hours section of the MEMBER PROFILE on the web site, or to record the hours manually to facilitate completing the Fraternal Activities Form at the end of the calendar year.
